To take advantage of U.O.C.P. nursing, it is required to give to the ANTEA Health Office Desk the following documents:
If the patient is at home:
a) GP’s request which declares:
“ The Palliative Care Operative Unit nursing is requested for Mr/Mrs (name, surname) diagnosed with (diagnosis)”
b) National Health document copy.
c) Hospital discharge paper copy and/or a copy of the clinical case first page, where is specified a recent clinical diagnosis
d) ANTEA application form to be filled in on the spot.
If the patient is nursed in a Hospital:
a) Hospital or Clinic request which indicates:
“ The Palliative Care Operational Unit nursing is requested for Mr/Mrs (name, surname) diagnosed with (diagnosis), now admitted at (sanitary structure, ward, bed to declare)”.
b) ANTEA application form fully filled in (ANTEA application form can be asked for by fax at 06/6620124).
These papers can be sent directly from the social service assistance of the hospital, or from the same patient nursing ward.
After receiving the papers, the U.O.C.P. Valuation Unit visits the patient at home or at the Hospital communicating the treatment headlines and needed assistance to the GP and the families , by two working days since the request.
